    Helpful Reminders From Nurse Moreno:

    Dear Parents (6th grade only),

Just a reminder to submit your student's updated shot record for ALL 6th graders going to 7th grade next school year (2023/24).

Per Texas state law, ALL students going into the 7th grade will need the following immunizations (or a conscientious exemption form) in order to start their 7th grade year on time.

    REQUIRED IMMUNIZATIONS:

    Meningococcal (MCV)

    TDAP - booster

    RECOMMENDED, NOT REQUIRED:

    HPV

*Your student may have already received these immunizations. If so, please send a copy of the updated shot record to the school nurse via email vmoreno@scuc.txed.net or the student may drop off to the nurse in person. The shot record will then be update in Ascender. Please do not upload shot records to Ascender, this will not reflect compliance for your student. I appreciate your cooperation with this matter.

This will assist in a smooth transition/start to your student's 7th grade year. Thank you!

Flu, common colds, and coughs are in season. Please remember to keep your student home if they have a temperature over 100 degrees, or if they have vomiting or diarrhea. They will need to be fever free for 24 hours without the use of medication, before returning to school. This policy is in the student handbook.

Please do not send any kind of medication to school with your child. This includes any over the counter medications (Tylenol, Motrin, allergy meds, cough drops, etc.). Please have an adult bring the medication and signed medication administration form to the clinic. Please refer to the Health Services Medication Policy for more information.

I would like to encourage parents to notify the clinic about any of the following:

*Change of phone number (cell or home)

*A change in your student's current health status

    * A newly prescribed medication

    * A change in current medication

    * A serious injury, illness or hospitalization

    * Fracture, sprain, stitches, or a cast

    * A contagious disease such as COVID, flu, strep throat, etc.

    * If your child is going to be absent for an extended period of time.

*Updated immunization record to be turned into nurse

You can find all necessary medication forms and action plans on the health services link provided here: Health Services Forms

Finally, if you have not filled out your student's Health Forms in Ascender, please do so as soon as possible. This is important for any health concerns your student may have that the nurse/teachers need to be aware of.
